If your child:

 

• has been seen by a doctor for a medical procedure, surgery, or injury, a doctor’s note will be required to return to school. The note needs to include dates of absence/s and any restrictions.
  ​

• requires medical equipment (crutches, boot, splint, or ace bandage) related to an injury, and a doctor’s note is needed.
   

• has been absent from school for 5 days or more, a doctor’s note will be required when returning to school.
